CPHC will drive the reform of the curricula of all professional degrees to mark Primary Healthcare as a key learning outcome, with the following highlights:

To inspire students to consider specialising in Family Medicine and understand collaboration with primary care doctors for continuous, seamless patient care

Health Informatics
CPHC will derive a comprehensive data integration platform to enhance the interoperability and accessibility of health data disciplines, with the following highlights:

To spearhead the use of data to enhance the understanding of primary healthcare

To enhance sharing and communication across disciplines and institutes to achieve a more holistic approach to healthcare

To enable seamless record linking between Western and Chinese medicine

To focus on unifying health and dental records, recognizing the essential connection between oral health and overall well-being
Medical
CPHC will work on informing and improving primary healthcare and coordinating multidisciplinary teams to provide inputs on clinical care, disease prevention and health promotion.
We will assist our affiliate Health Centre (DHC) and District Health Centre Express (DHCE) in the Central & Western District, Yau Tsim Mong District and Kwai Tsing District in strengthening their community networking (medico-social collaboration), providing staff training, organising public health talks, opening up new opportunities for undergraduate and postgraduate learning and facilitating research in the community.
Social Healthcare
CPHC will revolutionize the approach to chronic disease prevention and management, with the following highlights:

To promote community capacity building, outreach and collaboration in primary healthcare

To develop medico-social resource hub in the community and enhance collaborations with different stakeholders to improve the health of the general public
Public Health
CPHC will elevate primary healthcare practices and to drive impactful change across disciplines from a Public Health perspective, including:

To enhance awareness of global health issues and the development of skills such as negotiation, advocacy, and responsibility in the primary healthcare settings

To create platforms and coordinate with NGOs, volunteer groups, private sector organisations and community members for collective impact and better outcomes
Policy
To promote the development of primary healthcare on all fronts, CPHC will foster policy advocacy and innovations, and address critical research needs in support of the relevant public policies. Our actions include conducting public policy research that informs healthcare strategies, develop evidence-based models to help the government better ascertain our service needs, promote knowledge sharing and stimulate discussions among stakeholders, and devise evidence-based strategies to evaluate service outcomes.
