Community-based Healthy Ageing Initiatives

The initiative focuses on strengthening Hong Kong’s preparedness for an ageing population. It aligns with the World Health Organization’s advocacy to develop and maintain the functional ability of older adults, supporting healthy ageing across the life course.

In response to the growing number of elderly singletons and doubletons facing social isolation, the Jockey Club “Reach to Bridge” Community Elderly Support Project was launched to strengthen community support and enhance the well‑being of older adults in Hong Kong

The Jockey Club Pathway to Healthy Ageing Symposium was held on 22 February 2025, bringing together stakeholders from healthcare, social care, and District Health Centres to advance healthy ageing in Hong Kong. The symposium featured the sharing of findings from a territory‑wide functional assessment of older adults, alongside discussions on the impact of the Jockey Club Pathway to Healthy Ageing Project on older adults’ well‑being. The event fostered meaningful cross‑sector dialogue and highlighted the importance of collaboration in strengthening primary health care and aged care services.​
